SUPERBASS Tournament entry forms may be found in the SUPERBASS Magazine, at sponsor locations or downloaded from our website. Complete your entry form and mail.
We will only accept checks or money orders. NO CREDIT CARDS at this time, but I'm working on it!
Each entry form must be completely filled out and signed. Incomplete entry forms will not be accepted.
Be sure to enter the correct Division Information. Incorrect division information could result in your points being added to the incorrect division. All unidentified entry form division information will be added to the OPEN division.
The Entry Form has been set up in an Adobe.pdf file and for your convience has entry fields so that you may add your information & print it with all information entered. Please complete the entry form in legible Printing or Typed and mail to. Don't forget to Sign entry form!
